FONCTIONS ET RESPONSABILIT--S PRINCIPALES: Effectuer les activit--s d'inscription des patients, conform--ment aux protocoles du programme et aux proc--dures d--partementales applicables Proc--der aux activit--s de saisie des donn--es d'informations pertinentes re--ues -- diff--rents points de contact pendant la prestation des services du programme Fournir des r--ponses aux appels de renseignements g--n--raux soumis par t--l--phone ou sur des plateformes Web sur les services du programme, l'--tat de la maladie ou les questions li--es aux produits, en fonction des informations fournies dans le cadre du programme Fournir un soutien m--dical aux patients et aux prestataires de soins de sant--, y compris, mais sans s'y limiter, une --ducation sur des maladies et th--rapies sp--cifiques et sur la gestion des maladies; Pr--parer et assister -- des sessions de formation approfondies et sp--cialis--es pour des th--rapies sp--cifiques fournies par l'--quipe pharmaceutique Support t--l--phonique (y compris les appels de maintenance et le soutien de l'observance -- la fois entrant et sortant selon les besoins); Administrer et respecter les protocoles de prise en charge des cas, y compris la surveillance clinique, la collecte de donn--es, le suivi continu des patients, l'observance, la surveillance de l'interruption / arr--t du traitement, la notification des --v--nements ind--sirables et la liaison avec les fournisseurs de soins de sant-- primaires; Travailler directement avec le sp--cialiste du remboursement et le m--decin pour obtenir le financement des th--rapies de leurs patients; aider -- la recherche de financement alternatif au besoin; Assister -- des r--unions de patients, -- des conf--rences au besoin pour informer les patients, les prestataires de soins sur la maladie / l'information sur les produits et les services de gestion de cas; Doit --tre en mesure d'identifier et de signaler les EI -- Sant-- Canada, -- Med Info et -- entrer dans le syst--me source et les syst--mes tiers. Solide sens des affaires et des finances Solides comp--tences analytiques et math--matiques ___________________________________________________________________ PRIMARY DUTIES AND RESPONSIBILITIES: -Perform Health Care Practitioner and patient registration activities, according to program protocols and applicable departmental procedures -Process data entry activities of pertinent information received at different points of contact during the delivery of program services -Provide answers to general inquiry calls submitted via telephone or web-based platforms on program services, disease state or product related questions, based on information provided as part of the program materials -Provide medical support to patients and health care providers, including but not limited to education on specific diseases and therapies and disease management; -Prepare for and attend extensive, specialized training sessions for specific therapies provided by the pharmaceutical team -Telephone support (including maintenance calls and adherence support both inbound and outbound as needed); -Administer and adhere to case-management protocols, including clinical monitoring, data collection, on-going patient follow-up, compliance, therapy disruption/discontinuation monitoring, adverse event reporting and liaison with primary healthcare providers; -Work directly with the reimbursement specialist and the physician to secure funding for their patient's therapies; assist with alternative funding searches when needed; -Attend patient meetings, conferences as needed to educate patients, providers regarding disease/product information and case management services; -Must be able to identify and report AE's to health Canada, Med Info and enter into source system and third-party systems. -Maintains confidentiality of client and corporate information and discusses same only with appropriate personnel -Liaise with physicians' offices, pharmacies, services providers and other parties to provide comprehensive and coordinated service to patients and physicians and resolve issues as they arise; MINIMUM SKILLS, KNOWLEDGE AND QUALIFICATIONS: Registered Nurse with the College of Nurses of Ontario. Must be a member in good standing -5-7 years of related experience -Knowledge of Microsoft Word, Excel, PowerPoint and other Office Programs -Strong computer and software skills: Excel, Word, Outlook, Adobe -Excellent written and verbal communication skills in English and French is preferred -Effective interpersonal and leadership skills -Effective organizational skills; attention to detail -Ability to consistently meet deadlines -Excellent problem-solving skills; ability to resolve issues effectively and efficiently --Acrobat, CRM database entry, VOIP telephony, web portals, teleconferences, web-based meetings. -Strong business and financial acumen -Strong analytical and mathematical skills Career Level - IC - Business Support - B4 Worker Type Regular McKesson is an Equal Opportunity employer.
