Hospice Experience License: Must have a valid nursing license in
hand Certifications: CPR, BLS Must-Have: Proficiency in HomeCare
HomeBase and OASIS within the last 6 months Description: Provides,
coordinates, and directs the provision of home nursing care
according to physician's orders. Applies nursing processes
competently based on agency policies and procedures. Responsible to
Clinical Manager. Directly supervises and evaluates Home Health
Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
Performs home visits and participates in periodic office meetings.
Must have reliable transportation and a current driver's license.
Agency-required liability insurance is necessary. Average number of
patients per day: 5-8. Weekly points expectation: 30 points per
week. Contractor will be assigned between 5-10 patients per day,
equating to roughly 6-8 points. Types of cases include Start of
Care, Wound Care, and Follow-up Visits. Must be willing to travel
up to 75 miles from the designated care center. Mileage will be
reimbursed at the IRS standard rate. The contractor is compensated
from one patient home to another. The commute to and from the first
and last patient is not billable. Local candidates are accepted at
the same rate. Must be open to on-call rotations. All documentation
must be completed within 24 hours of the appointment with the
patient. The clinician is expected to accept every patient on their
schedule regardless of distance. There are no guaranteed hours with
this contract. The clinician must meet weekly productivity points
of 30. All overtime must be pre-approved. If the clinician is not
receiving enough patients each week, they may have the opportunity
to float to another nearby clinic to pick up patients. 5 Benefits
