Monday-Friday 8am-4:30pm /On-Call Weekend Commitment Monthly/
Quarterly Location: Coverage in Southern Johnson County Kansas.
Other Areas : Jackson, Platte, Clay, Cass Counties in Missouri
and/or Leavenworth, Johnson, Wyandotte, Franklin, Douglas and Miami
Counties in Kansas. The role you'll contribute: Administer nursing
care to patients in accordance with the physician's order and under
the direction and supervision of a Registered Nurse. Provides
intermittent, individualized nursing care, teaches and supervises
the patient and family members or caregivers to provide continuity
of care and to assist in implementing comprehensive care. Delivers
high quality nursing care, performs technical nursing skills with a
high degree of proficiency, and promotes the team approach in the
delivery of patient care. Executes physician's orders and notifies
RN/Case Manager of necessary updates or changes in condition.
Completes documentation according to Agency Policy and Regulatory
guidelines and performs home visits as ordered. Continued
employment with the agency is contingent upon your ability to
perform job duties as outlined in this job description. The value
you'll bring to the team: Other duties as assigned. Develops
professional and clinical skills through attendance at staff
development meetings, pertinent educational activities, and
professional affiliations. Participates in the review process and
the agency performance improvement program. Provides patient
education, medication administration, wound dressing changes, and
ostomy care as directed by the RN and ordered by the physician.
Displays flexibility in accepting assignment changes and
participates in weekend/after-hours coverage and call rotation.
Reports pertinent observations to the attending physician and RN,
and implements changes in the patient care plan as directed. The
expertise and experiences you'll need to succeed: Minimum
qualifications: High School Grad or equivalent Technical/Vocational
School- Graduate of an Accredited School of Licensed Practical
Nursing Active American Heart Association BLS Licensed Practical
Nurse (LPN) Preferred qualifications: 1 year of medical/surgical
experience IV Certified (IV) Valid in state Drivers License with
